IN THE MATTER OF K.J.C., YOUTH IN NEED OF CARE.

YOU ARE HEREBY NOTIFIED that a Petition has been filed in the above-entitled Court by the Montana Department of Public Health and Human Services, Child and Family Services Division (hereinafter CFSD), Drawer 310, Kalispell, MT 59903, requesting an extension of temporary legal custody of the above-named youth to the Department of Public Health and Human Services for a period of no more than six months; NOW, THEREFORE,

YOU ARE HEREBY DIRECTED to appear on the 3rd day of September, 2004, at 9:00 o'clock a.m. at the Courtroom of the above-entitled Court at the Courthouse in Kalispell, Flathead County, MT, then and there to show cause, if any you may have, why CFSD should not be granted an extension of Temporary Legal Custody of the above-named youth; as it pertains to the rights as birth father.

The name and date of birth of the youth are as follows: K.J.C. d.o.b. 07/07/1992

A copy of the Petition hereinbefore referred to is filed with the Clerk of District Court for Flathead County; and is hereby served upon you by publication at this time.
