A Petition by Janet Ann Shaw, born 08/30/53 in City of Grosse Pointe, State of Michigan now residing at 345 Hayes Gulch Road, Sagle, proposing a change in name to Janet Samadhi Rose has been filed in the above entitled court, the reason for this change in name being I am now divorced and no longer want my ex-husband's name.

The petitioner's father has died and the names and addresses of the petitioner's near relatives are:

Such petition will be heard at 9:30 o'clock a.m. on October 4, 2004, in courtroom number __ at the Bonner County Courthouse. Objections may be filed by any person who can, in such objections, show to the Court a good reason against such a change of name.
